Ensure that all teachers/assessment administrators have read the Test Directions
for Teachers.
If the precoded CTB Barcode Label for Multiple Assessments and the precoded
Student Information Form/Precoded Answer Sheets for CAT Plus have not arrived at
your school by the date of testing, administer the assessment as planned. The precoded
forms should be placed on or inserted as applicable with the correct answer document
after testing. If a student’s precoded CTB Barcode Label for Multiple Assessments or
Student Information Form for CAT Plus is inaccurate, or if one is not available, please
complete all information on the inside front cover of his or her student book.
If they are able to, students may complete the following information: student’s name,
teacher’s name, names of school and district, student’s grade, student’s birth date, and
student’s gender.
If students are not able to complete their portion of the identifying information,
complete all the student-identifying information, including the correct birth date, on
the inside front cover of the student books or on the precoded Student Information
Forms prior to distributing them.
Review all data for accuracy.
During Each Day of the Assessment
Implement Assessment Security procedures (page 3).
Distribute correct assessment materials to teachers/assessment administrators prior to
the start of testing.
Ensure that all students have the required materials.
Collect and account for all assessment materials immediately following testing.
Following the Assessment

Place CTB Barcode Label on designated area of Multiple Assessments test book.
Verify the student name written on the Multiple Assessments student book is the
same as the barcode label before the label is affixed. If students received Testing
Accommodations, ensure that the correct information is bubbled on the biographical
page of the student book (inside front cover).
Place Student Information Form inside CAT Plus test books. Students in grades 4–9
will have a precoded Answer Sheet. If students received Testing Accommodations,
ensure that the correct information is filled in (pages 28 and 33).
Please ensure that the completed Student Information Forms are placed in the
appropriate student books. Erroneous placement can cause incorrect scores or
invalidation of students’ tests.
Check Group Information Sheets (GISs). An accurate GIS must be placed on top of
each group of student books whose scores are to be reported together (pages 34–35).
Complete the School/Group List (pages 36–37).
