When you check this...
look for this:

TEACHER NAME
The assessment administrator’s last
name must be printed in the boxes,
followed by a space and then the first
name or initial, if needed. Under each
box, the circle with the same letter
must be filled in.

SCHOOL NAME
The school name has been precoded.

NUMBER STUDENTS
TESTING
GRADE
The number of students whose answer
documents are grouped with this GIS
must be printed in the boxes and the
corresponding circles filled in.
The circle that shows the grade of
the students being assessed should be
filled in.

SPECIAL CODES
This has been precoded with your
school identification numbers.

ORGANIZATION NAME,
ELEMENT/DISTRICT
NAME, SO #, STATE
Organization Name, Element/District
Name, and SO # have been precoded.
State name has been precoded if
applicable.
After a GIS has been checked for accuracy, place it on top of the
students’ answer documents. Then put the GIS and the answer
documents in the appropriate envelope.
